Scholarship Award in Vision and Medicine (SAVM)

Application Process

We encourage you to please review the entire application before you begin completing your application. Time spent reviewing the instructions will be valuable in supporting your efforts to submit your best work.

Applicant must PRINT AND MAIL completed Sections 1 through 3 (MS Word format) along with your official high school transcript and SAT and/or ACT scores by April 1, 2010 to:

Sections 4 through 8 (Adobe PDF format) must be SUBMITTED ONLINE by April 1, 2010. PDF Documents are in Adobe Portable Document Format. To view or print them, you will need Adobe Acrobat Reader 7.0 or higher, which can be downloaded free of charge from Adobe.

We urge you to read the application instruction before completing the application form so as to submit a strong application.

SAVM scholarship recipients must enroll in and attend an accredited college or university in the academic year after their selection and provide verification of enrollment. Scholarship funds are paid directly to the schools and no funds will be dispersed to students directly.

Scholarship recipients are selected based on:

  • Interest in science or medicine
  • Essay “The Influence of the History of African Americans in Ophthalmology on My Role in Improving Healthcare for African Americans and Others”
  • Academic achievement
  • Leadership
  • Financial need
