Registration Fees
You may register several ways
Online using our secure server and a credit card. Downloading a pdf file and mailing or faxing to:

Special early fees - received on or before July 15
(post-marked date):
- $270.00 for physicians
- $210.00 for other health professionals and physicians-in-training
If you are a member of MALTCP, the fees are:
- $240.00 for physicians (if membership dues are paid for 2008)
- $180.00 for other health professionals and physicians-in-training (if membership dues are paid for 2008)
Regular fees - if received after July 15:
- $335.00 for physicians
- $275.00 for other health professionals and physicians-in-training
If you are a member of MALTCP, the fees are:
- $310.00 for physicians (if membership dues are paid for 2008)
- $250.00 for other health professionals and physicians-in-training (if membership dues are paid for 2008)
If you are attending ONLY the PreConference Interactive
Session:
Social Worker Credit
An additional $10.00 fee is charged for social worker credit. This fee must be submitted with the registration form.
Payment Methods
Fees may be paid by cash, check, Discover, MasterCard or VISA.
Cancellation/Refund Policy
A full refund of fees, less a $50 administrative fee, will be made if notice of cancellation is received, in writing, by Friday, August 8, 2008. No refunds will be made after August 8. Please mail or FAX to the address listed on the registration form, or e-mail rogerse@health.missouri.edu.
