Early registration fee (received on or before July 1, 2009):
Practicing Physician: $375.00
Resident Physician or Nurse: $275.00
Registration fee (on and after July 2, 2009): - All Practitioners: $500
An Obstetric Emergencies Workshop is being held on July 31, 2009 with an additional registration fee of $500.00.

PAYMENT METHODS:
Registration fees may be paid by cash, check or credit card (Discover, MasterCard or Visa). The check should be made payable to the University of Missouri.
SUBSTITUTION POLICY:
If you register but cannot attend, you may substitute another individual from your organization. A substitution must be made in writing and submitted to the CME Office prior to the conference.
CANCELLATION/REFUND POLICY:
A full refund of fees will be made if a notice of cancellation is received, in writing, by Friday, July 17, 2009. A $100.00 administrative fee will be assessed for all cancellations received after Friday, July 17, 2009.
