Registration Fees
For the Frail Elderly Conference:
• Special early fees - if received on or before July 24 (post-marked date):
- $270.00 for physicians
- $210.00 for other health professionals and physicians-in-training
• If you are a member of MALTCP, the fees are:
- $240.00 for physicians (if membership dues are paid for 2009)
- $180.00 for other health professionals and physicians-in- training (if membership dues are paid for 2009)
• Regular fees - if received after
July 24:
- $335.00 for physicians
- $275.00 for other health professionals and physicians-in-training
• If you are a member of MALTCP, the fees are:
- $310.00 for physicians (if membership dues are paid for 2009)
- $250.00 for other health professionals and physicians-in-training (if membership dues are paid for 2009)
Social Worker Credit
An additional $10.00 fee is charged for social worker credit. This fee must be submitted with the registration form.
Payment Methods
Fees may be paid by cash, check, Discover, MasterCard or VISA. To pay by credit card, please fill out the appropriate section of the registration form and sign where indicated, or online via secure server.
Cancellation/Refund Policy
A full refund of fees, less a $50 administrative fee, will be made if notice of cancellation is received, in writing, by Friday, August 14, 2009. No refunds will be made after August 14. Please mail or FAX to the address listed on the registration form, or e-mail rogerse@health.missouri.edu.
Substitution Policy
If you register but cannot attend, you may substitute another individual from your organization. A substitution must be made in writing and mailed to the address listed on the registration form.
