Fees may be paid by cash, check, Discover, MasterCard, or VISA. The check should be made payable to the University of Missouri-Columbia. To pay by credit card, please fill out the appropriate section of the registration form and sign where indicated.
Registration Fees
$150 Special Early Registration Fees – if received on or
before September 12, 2008.
$165 Regular Registration Fees – if received after
September 12, 2008, but on or before October 6.
$180 Late Registration Fees – if received afterOctober 6, 2008.
$75 Special rate for students, residents, and fellows
(Registration cannot be done online, it must be faxed to
Kara Carr at 882-5666) .
All conference materials and catered functions are included.
Substitution Policy
If you register but cannot attend, you may substitute another individual from your organization. A substitution must be made in writing and submitted to the CME office.
Cancellation/Refunds
A
full refund will be made if notice of cancellation is received, in writing, by Friday, September 26. A $50 administrative fee will be assessed for all notices received after September 26.
